6545 Application note

In one of the old Micro6502 or 6502 User Notes newsletters there was reference to a "recently released" application note for the 6545 CRT controller chip.

I have not been able to find a scan of this online anywhere.

Does anybody where I may find it?

I wish to use 6545 with the 6502SBC that I built (based on Daryl Rictor's SBC2.5 design).
