Updated version 3.04 now completed... yay!

Updates include:
- Separate source files for the Base, CCM, PEM and SIM.
- Some enhancements to multiple routines, shorter and faster code execution.
- SIM (now 1.24) supports the Reader/Punch devices to the second UART port.
- SIM module is shorter code, eliminated a routine that wasn't really required.

This version is mainly to provide an updated version for my prototype 3.3V system based on the Pocket SBC V2.
The updated hardware is all 3.3V logic, uses a NXP SC28L92 DUART and replaces the Compact Flash Card with a Hitachi MicroDrive.
An updated BIOS/Monitor supports the new hardware and provides the ability to turn on the write caching on the MicroDrive.
Note: The updated BIOS should provide proper support any for any IDE disk drive as all BIOS calls use standard IDE commands.
