I came across this source listing for the Kim-1 Enhancement PROM dated 1977.
15 pages plus symbol tables etc.
It looks like it is by Rene Vega of Detroit September 1976

Hopefully, that wasn't one of the originals from the famous "jar" of first runs that were for sale.
Only the top layer of chips worked as legend has it!
